See 2005 Islamic Games article. Seems like a mix tournament of full and youth/B national teams until it became strictly u-23 from the next edition 2013. See the 2005 tournament brochure (page 17) stating there is no age limit.
For Pakistan I can fully confirm it was the full national team, and so was Malaysia as clearly the squad and coach is full strength, Malay press from 2005 also treat it as the national team

Apart from the obvious contradictions and evidence that I mentioned about North Korea in my previous thread, this page from a Bangladeshi magazine from 15 May 1985, shortly after the conclusion of this tournament explicitly states that only Bangladesh and Nepal (apart from Pakistan Greens) were the full national teams. And the North Korean and Indonesian teams were not.
Here is a local Indonesian website listing all matches from their A-team including non-FIFA matches against club sides or youth team. The fact that the Quaid Azam Cup matches don't appear on that list let us conclude that it was an understrength team present in Pakistan. The RSSSF list of Indonesia which also includes several matches against clubs or selected sides also omits these 1985 matches.
According to this Indonesian blog, it explicitly mentions that the Indonesian team participating in this tournament was the youth team, mainly consisting of the squad which won the 1984 Asian Schools Championship.
A follow up on this, I managed to get hand of the full lineups of Bangladesh vs the North Korean team (1985 Quaid-e-Azam International Tournament). Not sure if it's conclusive to determine the status of the North Korean side.
